‘ABCD Plus’ refers to Cardiovascular Disease risk factors: Atrial Fibrillation (heart rhythm problems), high Blood pressure (hypertension), high Cholesterol (hyperlipidaemia), and Diabetes; and health behaviours such smoking and diet.

Wales faces a significant health challenge, the growing burden of cardiovascular disease (CVD), which particularly affects the most deprived communities and drives health inequalities.
