Eligibility for participation in CMP for Wales
All Children who have their fifth birthday between September 1st and August 31st of the current school year , who also attend a school in Wales (Reception Cohort).
CMP Schedule forms are lists of children at a particular school with space to record all the required results for the Child Measurement programme for Wales. Inclusion of a particular child on the CMP schedule list is dependant on having a correct school code linked to the child’s record .the schedule form also contains details of the last two recorded height/ weight /BMI Centile results.
Blank CMP unscheduled forms are used to capture results and update the child health system concerning children not listed on the schedule form.
