This toolkit is designed to help those working in or with primary care to consider inequalities in all areas of planning and/ or implementation of services, programmes, or specific and targeted interventions.
The equity checklist for anyone who is involved in the planning and delivery of primary care services including but not limited to cluster leads, cluster coordinators, primary care services lead.
A rapid reminder to help primary care colleagues keep health equity in mind in any piece of work under consideration.
