Primary care toolkits

The primary care toolkits provide an array of guidance and resources for people working with and within primary and community care in Wales.

  • Healthcare Public Health Framework toolkit

    This toolkit brings together a number of tools and frameworks which could be used alone or in combination to support healthcare settings, systems, and partnerships to assess and optimise their services with a population lens. The toolkit is a resource to aid in the identification of trends, needs, and disparities within communities, and to support effective planning and delivery, making the most efficient use of resources. The tools it contains are not intended to be prescriptive but to act as a guide to allow decisions to be taken in a systematic way.

  • Accelerated Cluster Development toolkit

    The Accelerated Cluster Development (ACD) Toolkit is a collection of information and adaptable resources for stakeholders who will be supporting and driving forward the implementation of the ACD programme including Primary Care teams, Professional Collaboratives, Clusters and Pan Cluster Planning Groups. The toolkit will enable stakeholders to learn more about the ACD programme, its components and access tools to support local delivery. The toolkit is designed to translate theory into practice and be a useful information and resource portal.

  • Community Infrastructure Programme

    The Community Infrastructure (CI) Toolkit is a collection of information, and resources, designed for anyone (inclusive of profession, area, or level of practice) supporting the delivery of place-based care, regardless of setting.

  • Multi-Professional Framework for Integrated Working

    The aim of the multi-professional framework is to support organisations to deliver joined up, outcome focused, evidence-based community services, through a place-based multi-professional working model of care.

  • Urgent Primary Care Centres (UPCC)

    The Urgent Primary Care Centres (UPCC) toolkit is a collection of information and resources for stakeholders who may be supporting and driving forward the Urgent Primary Care Centres or for colleagues who have in interest in the work being carried out.

  • Healthy Days at Home Wales

    Healthy Days at Home Wales (HDAHW) is a suite of measures designed to demonstrate ways of measuring health and care systems across Wales. These can be adapted to meet the needs of the user and provide heatmaps and basic analysis of population interactions with specified health and care providers.

  • Clinical Governance Practice Self-Assessment Toolkit (CGPSAT)

    The Clinical Governance Practice Self-Assessment Toolkit applies to all General Practices in Wales, which includes all General Medical Services Contractors (GMS), Alternative Providers of Medical Services (APMS) Practices and GP (General Practitioner) practices managed by Health Boards.
