  • Understanding social prescribing in Wales: a mixed methods study 2021– There are multiple models of social prescribing working in Wales with practitioner roles based in places such as local authorities, third sector, housing, higher education, and primary care; and social prescribing activities most commonly based within third sector and community spaces. The aim of this work is to understand the landscape of social prescribing activity across Wales, and where possible, to quantify and describe its functions

  • The primary care needs of people living with overweight and obesity in Wales (summary) 2021 – this healthcare needs assessment (HCNA) provides an overview of the primary and community healthcare needs of adults of working age (18 – 64 years old) in Wales who are living with overweight or obesity.

  • Primary Care Obesity Prevention Action Plan 2022-2024 – This initial two-year action plan, which covers April 2022 – March 2024, is designed to support the implementation of the primary and community care elements of the adult All Wales Weight Management Pathway (AWWMP), in line with the Healthy Weight Healthy Wales (HWHW) Delivery Plan 2022-24. 
  • All Wales Diabetes Prevention Programme Intervention Protocol 2022 – The All Wales Diabetes Prevention Programme (AWDPP), led by Public Health Wales, offers targeted support to people who are at an increased risk of type 2 diabetes, with the aim of preventing them from developing this condition. This document supports the consistent implementation of the AWDPP.
  • Social Prescribing Interfaces 2022 – Through understanding the specific interfaces between social prescribing, health and care services, and wellbeing activities and community assets, this paper aims to inform Social Prescribing Interfaces 5 strategic direction and policy development, to support and improve the wellbeing of both current and future generations in Wales.

  • SPPC Fund 2022-2023 Obesity Prevention Projects Benefits and Outcomes 2022 – The purpose of this paper is to highlight the benefits and outcomes that are anticipated from the obesity prevention projects. Ten projects to address obesity prevention are being developed across the 7 health boards.
