Aneurin Bevan UHB consists of 11 clusters – Caerphilly North, Caerphilly South, Caerphilly East, Blaenau Gwent West, Blaenau Gwent East, Torfaen North, Torfaen South, Newport West, Newport East, Monmouthshire North, Monmouthshire South.

There are 14 Primary Care Clusters within the Betsi Cadwaladr University Health Board (BCUHB) area.
Locality Pen Profiles
Locality Pen Profiles have been developed as part of the North Wales Regional Partnership Board’s Community Services Transformation Programme, and are intended to support the development of place-based strategic planning and commissioning for health and care, across the region.
Based largely on GP cluster boundaries, locality Pen Profiles bring together data and insights from different agencies to build up a rounded picture of the needs and strengths of different communities at a very local, granular level. These insights are being used to help inform health and social care priorities for localities/clusters. This data will also be used on an ongoing basis to adjust priorities and enable partners to hold one another to account for delivering their ambitions.
BCUHB Cluster Population Health Directory
The North Wales Population Health Directory (NWPHD) focuses on the public health priorities and provides information on population health need at different geographical levels (including at Cluster level) to NHS staff for planning purposes.

Cardiff and Vale UHB have three localities and each locality has three clusters that sit within it.
Cardiff North and West Locality (intranet – only accessible via the NHS Wales network)
Cardiff South and East Locality (intranet – only accessible via the NHS Wales network)

Cwm Taf Morgannwg University health board (CTMUHB)
Cwm Taf Morgannwg University Health Board area is comprised of eight Clusters – Rhondda; Taff Ely; Merthyr; North Cynon; South Cynon; Bridgend East; Bridgend West, and Bridgend North.
Cwm Taf Morgannwg UHB patient population is approximately 450,000

Hywel Dda UHB consists of 7 clusters
Carmarthenshire: Amman Gwendraeth, Llanelli, Taf / Tywi (2Ts).
Ceredigion: North Ceredigion, Teifi and South Ceredigion
Pembrokeshire: North Pembrokeshire and South Pembrokeshire.

Powys teaching health board
Powys Teaching Health Board is responsible for improving the health and wellbeing of around 139,174 people living in Powys and for enabling the provision of high quality, effective healthcare services. There are 3 clusters within PTHB which work closely with 16 medical practices, 24 dental practices, 20 optometrists and 23 pharmacies.
Powys has made a distinction, since 2015 between clusters as planning mechanisms that span organisations, services and professions. Also there are GP networks as groups of general medical practitioners. This allows GP Practice issues and wider cluster planning issues to be discussed separately, but with one informed by the other.
All 3 Powys clusters have multi-disiplinary and multi-organisational membership including Health Board, County Council, Third Sector, Dentistry, Optometry and Community Pharmacy.

Clusters:
Swansea Bay University health board (SBUHB)
There are 8 cluster networks with the Swansea Bay University Health Board area and the populations vary from around 30,000 to over 75,000. There is clear leadership within the Clusters from GPs in particular and they have been developed on a multi professional basis. They now provide a structured mechanism for organising and delivering many community health and social care services, and have developed strong links with the third sector and other key partners. The Health Board’s Primary and Community Services Unit, is one of six Operational Units covering all of the clinical services provided by the Health Board, which leads on this agenda through it’s Cluster Development Team.
