- Cluster Development Framework 2022 – The purpose of this document is to outline the cluster development framework (CDF) as a tool supporting monitoring (quality management) and evaluation of Primary Care Model for Wales (PCMW) and Accelerated Cluster Development (ACD) implementation.
- ACD Transition Year Peer Review 2022 – The Peer review provides the opportunity for the hard work of clusters and their successes to be recognised, and to identify any barriers to service improvement.
- Primary care cluster workforce planning – HEIW (nhs.wales) – HEIW Primary Care Cluster workforce planning resources and guidance.
- ACD Planning Guidance – This guidance set out the requirements for the development of the PCPG strategic three year plan and the associated cluster plans during the 2023/2024 foundation year.
- ACD Supporting Governance (Model paper for health board use) 2023 – This paper set out the key governance principles, overarching legislation and suggested governance for consideration by health boards in developing and implementing the planning and delivery mechanisms described in the Accelerated Cluster Development programme.
- Optometry professional collaborative pilot (CTMUHB) Summary Report 2023 – This report describes the findings of the Strategic Programme for Primary Care sponsored Optometry Professional Collaborative Pilot.
- Peer Review Primary Care Clusters 2022-2023 – In 2022/23 a series of peer review meetings was held to assess progress of the Cluster model and to inform a more formal peer review process going forward. This report summarises the learning from these discussions and provides recommendations for the further development of the Cluster Peer Review Programme.
- New Service Developments Guidance for Engagement and Consultation 2023 – This document provides guidance to encourage engagement to support new service developments, identifying service gaps, developing service improvement proposals and managing change.
- Cluster Self Reflection 2024 – Cluster self-reflection is one of three agreed methods for evaluating the progress of cluster working towards the Primary Care Model for Wales (PCMW) and Accelerated Cluster Development (ACD).
