Primary Care One
Primary Care One provides an overview of the strategic direction of primary and community care in Wales, the Primary Care Model for Wales, and an overview of the roles and responsibilities of professional collaboratives, multi-professional clusters and pan-cluster planning groups. With the aim of sharing information, raising awareness and influencing action, to improve the health and well-being of the population of Wales.
Primary Care One provides professionals, with information, resources and signposting to:
- support development of multi-professional working
- strengthen professional collaboratives and cluster working
- enable collaboration across partnerships
- inform the planning and delivery of services that meet population need
- facilitate sharing of innovation and practice

The Strategic Programme for Primary Care (SPPC)
The Strategic Programme for Primary Care (SPPC) is an All-Wales Health Board-led programme that works in collaboration with Welsh Government working in partnership with the Primary Care Division within Public Health Wales.

The model for primary and community care, describes how care will be delivered locally, now and in the future, as part of a whole-system approach to deliver ‘A Healthier Wales’.

A health transformation programme to deliver better outcomes through integrated services in the community.

Cluster working
A cluster brings together all local services involved in health and care across a geographical area, typically serving a population between 25,000 and 100,000. Working as a cluster ensures care is better co-ordinated to promote the wellbeing of individuals and communities.

Allied Health Professions (AHPs) are 13 individual professions in Wales regulated by the Health and Care Professions Council (HCPC). Primary and Community Care Compendium of roles and models (PCCC)
A once for Wales digital platform for the sharing of innovative and unique models of primary and community care provision. It has been produced on behalf of the Strategic Programme for Primary Care (SPPC) in collaboration with Health Education and Improvement Wales (HEIW).

Primary care toolkits
The primary care toolkits provide an array of guidance and resources for people working with and within primary and community care in Wales. Including Healthcare Public Health Framework, Clinical Governance Practice Self-Assessment Tool (CGPSAT) and Multi-Professional Framework for Integrated Working.

Public Health Wales Primary Care Division programme of work
The Primary Care Division in Public Health Wales leads national programmes of work to support the development of a sustainable health and care system focussed on prevention and early intervention; reducing inequalities and inequity in health and care; environmentally sustainable health and care, tools to support cluster working and the leading the national evaluation of the Primary Care Model for Wales and leading dental public health in Wales.
