Introduction

The purpose of the Primary Care Clusters Dashboard is to provide a user led Primary Care Cluster population health dashboard linked to primary care public health information which will support a range of users and inform planning conditions and be our flagship local area output.

Main points

  • The dashboard looks at list sizes, age and sex breakdowns, breastfeeding, low birth weight, mortality, avoidable mortality, chronic conditions and emergency admissions at primary care cluster level.
  • We have also produced an indicator summary tool, allowing users to visualize key indicators for all clusters within a health board simultaneously. Users can also compare indicator data with health board and Wales-wide averages. Additionally, a brief narrative highlighting key findings from the data is included.
  • A Primary Care Cluster is made up of groups of GP practices and are charged with working together and with partners to meet local need.
  • Primary Care Clusters are not an exact geographic area. LSOAs have been mapped to the cluster where the highest number of residents are registered to a GP practice within the cluster. 
  • Clusters for resident data are as at October 2024, using Census 2021 LSOAs.
